By reporting incapacity for work early – after 30 days of incapacity for work or after recurring short absences due to illness over a period of six months – you actively contribute to a reduction in disability cases.
Please also enclose any existing medical certificates and daily allowance statements with the form, so that we can process any exemption from contributions as efficiently as possible for the benefit of both the employer and the employee.
